    BOTOX – It’s Multiple Health Benefits

    Most people wouldn’t think of Botox as the go to treatment for some of the biggest medical problems plaguing people today. When most people hear the word “Botox” they invariably think that it is only for the Hollywood elite. The truth is, however, that with over 2 million Botox procedures performed each year in over 20 countries worldwide, Botox has clearly become main-stream. In addition, using Botox to reduce facial wrinkles was a happy accident discovered while it was being used medically to treat patients who had uncontrollable spasms and twitching of their eye muscles.

    It may also come as a big surprise to learn that the active ingredient in modern Botox was actually discovered over 100 years ago! Because Botox works by disconnecting the communication between our nerves and muscles so that specific muscles essentially become paralyzed, the use of Botox for the effective treatment of medical problems involving human muscles has been around for quite a while.

    So you’re probably wondering in what kind of situation would it be necessary to paralyze muscles RIGHT? There are certain medical conditions where the muscles become “over-active or spastic”, because there has been a breakdown in communication between the brain and the muscles due to a problem with the nerves. The use of Botox in these situations is like turning off the power switch to specific muscles that have gone rogue.

    The following is a list of medical conditions that have been effectively remediated by the introduction of Botox injections as a medical treatment:

    MUTIPLE SCLEROSIS

    For patients who suffer from multiple sclerosis, the communication between muscles and nerves begin to break down when the insulation surrounding the nerves begins to thin out, which leads to the muscles “doing their own thing a lot of the time”. This is called spasticity and because it is uncontrollable it can lead to crippling situations. In over-active-or spastic muscles, the muscles begin to “cramp” up. This can be extremely painful and debilitating.

    To put it into perspective for you, I am sure that at some time or another, without any warning at all, you have had an awful cramp in your calf or foot. Spasms like this can incapacitate a 300 lb. professional athlete in a second on the football field. There are over 650 different muscles in the human body. So imagine this happening to some of the other muscles in your body without any warning or control. This is what happens to people with Multiple Sclerosis and with each attack the insulation of the nerves is slowly being lost inside the body.

    Fortunately, medical professionals have found that Botox can produce a releasing effect on the cramping muscles and help Multiple Sclerosis patients by paralyzing the muscles that are causing the problem. If the muscles causing the problems can be identified with a small electrode, they can be targeted and injected with Botox and “turned off”, or paralyzed. The cramping and spasticity stops and the patient can breath a sigh of relief.

     

    OVER ACTIVE BLADDER

    In addition to having a physically crippling affect, there are some muscle malfunctions that have the potential for crippling a person’s social life. Our muscles all have different functions on both the inside and outside of the body. Our bladder is essentially one large muscle that squeezes like a water balloon when we urinate. This process takes precise and coordinated communication between our brain and the bladder. This is how we are able to “hold” it without having accidents in awkward situations. However, for some people the bladder becomes “spastic” and miscommunicates with their brain, which can cause the bladder to leak unexpectedly.

    It is estimated that 1 in 6 adults have some version of an overactive bladder. Botox can be used to calm an overactive bladder and allow people to return to a normal lifestyle.  It requires a simple procedure during which a catheter is inserted and Botox is injected directly into the bladder.

    Other options for treatment are oral medications that come with potentially harmful side effects and very invasive surgical procedures that have not yet been perfected. The benefit of Botox is that it offers a simple, effective solution with minimal side effects and lasts 6 to 9 months.

    HYPERHYDSOSIS or “Over Sweating”

    Perhaps one of the best ad slogans ever created was “Never Let Them See You Sweat”. And now thanks to Botox being approved for the treatment of Hyperhydrosis, there are going to be a large amount of sufferers saying those words with pride.

    Hyperhidrosis affects over 8 million people and is a condition that causes excessive sweating of the armpits, hands, feet and other areas of the body. Most people only sweat when they are working out, fearful, or anxious. But in patients with Hyperhidrosis, it happens all the time. This can be very uncomfortable and embarrassing. It can also affect business and social situations by giving a false impression of nervousness, uncleanliness, and/or anxiety. Most sufferers are too embarrassed to seek medical treatment for their condition or do not even know it exists.

    Similar to its other uses, Botox’s effectiveness in decreasing excessive sweating was only discovered accidently when areas treated with Botox for other reasons were noted to have decreased sweating as well. For Hyperhidrosis patients, Botox is injected into targeted areas, which disconnects the nervous system from communicating with the sweat glands.

    As with the other procedures, Botox typically takes a few minutes to inject with little discomfort. It is a very safe and effective treatment with little known side effects. The treatment will start to take effect in 5 to 7 days and will last 6 to 9 months.

    Botox has also been approved for the treatment of other medical conditions such as chronic migraines and jaw pain. Patients absolutely love the results and the word is spreading: The relatively quick and effective relief from several debilitating medical conditions makes Botox a minor medical miracle.

    “One of my earliest childhood memories is of greeting a new friend, shaking their hand, and having them recoil in disgust and say “Eeeeuwww” because the palms of my hands were so sweaty.

    I now know that there is a term for my condition: Hyperhidrosis. I have spent most of my life ashamed and embarrassed by my excessive sweating. It affects my armpits, hands, my feet and sometimes even my groin area. I know, gross.

Crazy…  If I am talking on the phone with someone, sometimes I hang up and discover that I am sweaty all over. I meditate often and find that even then, when I am calm, I’m sweating. There are the occasional times when I am bone dry- like when I first wake up.

    About 7 years ago I had a horrific car crash and almost died. I had to have several surgeries and one of my plastic surgeons brought up the use of Botox for my Hyperhydrosis. At that time my insurance was paying for it so I eagerly tried it out. After about 20 minutes and about 30 injections under each armpit – which was a little bit painful, but a small price to pay as I saw it- I was on my way home and had just a little tenderness. Several days later I noticed that my shirts weren’t getting big sweat stains! I actually felt more comfortable. The results were amazing.

    Flash forward a few years and Dr. Motykie, asked if I would like to be on the Marie Show as a patient with Hyperhidrosis, and to have Botox injections under my arms.     Cost Components

    The cost of a cosmetic surgery procedure will vary depending on how simple, brief, or elaborate the procedure is. For instance, the cost of a breast augmentation or tummy tuck surgery will differ from the cost of a brow lift or lip augmentation procedure. Some procedures involve general anesthesia, while others may involve IV sedation, local anesthesia or simply no anesthesia. The fee of anesthesiologist will influence the overall cost of the procedure.

    Even within the same procedure, the extent and complexity of treatment may differ. A liposuction surgery may be performed on a single body part such as the waist area, or it may be performed on the waist, hips, thighs, arms, and neck region. In some cases, the liposuction may be performed over a period of two days. This may also include the costs of overnight stay at the operating facility. Geographical factors will also impact the overall cost of surgery. A surgery in Beverly Hills, West Hollywood, and Los Angeles may cost differently from a similar surgery in Atlanta or Dallas because of the different cost of living index in different regions.

    The skills, experience, and reputation of the plastic surgeon who performs a procedure will usually determine the surgeon’s fee.      Brazilian Butt LiftFor several decades, women have requested buttock enhancement to fulfill their desire for a curvier backside. In the past, the most common procedure was the insertion of an implant into the buttocks that was similar to the modern breast implant. This surgery carried a high risk of complications and infections, and thus buttock implants never gained mainstream popularity.

    Luckily, a new alternative has been championed over the last several years known as the Brazilian Butt Lift. This procedure involves modern liposuction technology and techniques that transfer a patient’s own fat to their buttock area. In essence, a Brazilian Butt Lift consists of two procedures in one: liposuction and fat grafting to the buttock.

    The Brazilian Butt Lift is therefore a “two-for-one” procedure because it enables both body contouring through removal of fat from unwanted areas of the body while also adding significant volume to the buttocks.     What is the Procedure?

    The surgery takes approximately two to three hours. The first part of the surgery is liposuction to harvest fat for unwanted “trouble” areas such as the thighs and stomach.  Dr. Motykie uses the latest liposuction technology that preserves the survival of the fat. The fat is then purified and prepared for re-injection I to the buttocks. Therefore, the areas where the fat is removed will become slimmer and your buttocks will be augmented and lifted.

    Typically, it is estimated that 70 to 80 percent of the fat cells injected into the buttocks will survive if the fat is removed properly and re-injected by an experienced surgeon that is using modern techniques. Due to these high survival rates, the fat cells will thrive long-term in Dr Motykie’s patients. The final results of the fat grafting procedure can usually be seen within three months after surgery during which time the definition and final shape of the buttocks will begin to appear.

    Will Exercise Ruin my New Buttocks?

    Dr. Motykie specifically targets and removes fat cells from stubborn areas of the body where losing fat is typically difficult for the patient. Surgeons refer to these fat cells as having “institutional memory” because they are resilient to dieting and have higher survival rates when transferred to a different area on the same body.  In other words, fat cells that are transferred from stubborn “trouble” areas will still think they are in the thighs or waist even though they have been transferred to the buttocks. These “grafted” fat cells remain “stubborn” and maintain their volume long-term. Therefore, if you exercise on a regular basis you can still lose weight and tone other areas of the body without jeopardizing your new curvy backside.

    What Should I Expect During Recovery?

    Dr. Motykie recommends a recovery time of about two to three weeks following a Brazilian Butt Lift during which time he asks his patients to refrain from sitting on the fat grafting for excessive amounts of time as well as avoiding “fat burning” exercises.  Patients are given a compressive garment to wear for several weeks after surgery in order to help reduce swelling and bruising. Bruising usually disappears sometime in the first several weeks following surgery but the swelling takes approximately several months to subside completely. During recovery, the buttocks will continue to “shape-up” while the areas where liposuction was performed will continue to shrink along with the resolution of post-operative swelling.

    Where should you have your plastic surgery procedure performed?

    Not all surgeries should be performed in a hospital. In fact, the majority of elective plastic surgery procedures are performed in an office-based facility. If your surgeon is going to perform your procedure in one of these types of facilities, you should check at least the following in order to make sure your surgery is being performed in a state-regulated, safe surgical facility.
    1.    The Facility: Where your surgical procedure is going to be performed should be accredited and in good standing with at least one of the major accredited organizations.

    Using Facial Fat Grafting, Fat Transfer or Adipose Stem Cell Injections in Facial Reshaping

    The Use of Fat Grafting in Facial Reshaping
    Patients who are unhappy with their facial features are often turning to facial fat grafting rather than undergoing complex and costly reconstructive surgeries.  Facial fat grafting is now commonly used in place of injectable artificial fillers and silicone facial implants because most patients have an abundant supply of donor fat tissue and the procedure is essentially scarless.

    Also, Facial Fat Grafting is frequently combined with other facial rejuvenation and aesthetic procedures such as face lifting, lower eyelid surgery and rhinoplasty.  Some common facial motifs that can benefit from facial fat grafting are the gaunt face, long face, bottom-heavy face, chubby face, asymmetric face and the aging face.

    Patients often turn to facial fat grafting as an alternative to undergoing more invasive surgeries because fat grafting can yield a very natural result with a minimal amount of downtown for recovery.

    What Is Facial Fat Grafting?
    Facial Fat grafting (fat transfer, fat injection or adipose stem cell grafting) is a medical procedure that uses a patient`s own fat to rejuvenate or augment certain areas of the face.

    This fat is harvested from a donor site (such as the abdomen, waist, inner thighs or inner knees) utilizing a small syringe or blunt cannula. The fat is purified in a centrifuge in order to separate the blood, fluid and damaged fat cells prior to its use in facial grafting. The fat is then placed in small syringes and injected into the predetermined areas of the face.

    Facial Fat Grafting in Combination with Face lifting (The Volumetric Facelift)
    Facial aging is related to not only loose skin in the face and neck, but also to a loss of facial fat volume over time. As we age, we lose the fat underneath our eyes and in our cheeks which can lead to a gaunt, tired look even though we feel very much young and alive. A face lift is used to tighten and remove the excess skin in the face while fat grafting is utilized to replace the fat that has been lost during the aging process. The fat that has disappeared with age is easily replaced to create the most natural and youthful facelift result available from plastic surgery today.

    Because the fat graft material contains stem cells it allows for a minimally invasive rejuvenation procedure that gives the plastic surgeon the ability to sculpt and restore an aging face to a more youthful appearance.

    Fat grafting is an extremely versatile plastic surgery modality. Once the fat has been harvested and purified, it can be used to accomplish aesthetic facial reshaping by accentuating the cheekbones, filling in facial defects or scars, plumping the lips, camouflaging dark circles underneath the eyes and improving overall facial contour Fat grafting can also be used to take the place of facial implants without the associated risks of implants such as migration, scarring, infection and extrusion.

     

    Facial Reshaping Is Truly Sculpting Facial Features
    Throughout the evolution of plastic surgery, facial reshaping has traditionally been accomplished with reconstructive procedures and placement of silicone facial implants that can become very complicated surgeries with extended recovery periods.

    With the recent popularity of artificial fillers such as Restylane and Juvederm, more attention has been directed to restoring facial volume. Although many doctors encourage patients to use these products to reshape the face, patients need to be aware that these corrections are temporary, and often several syringes are needed to fill in larger depressions. This can lead to considerable expense over time that may end up costing more than having facial fat grafting in the first place.

    Facial fat grafting offers an alternative to facial fillers for those patients that do not want to spend a lot of money on temporary artificial fillers that require yearly or bi-yearly maintenance. The main advantage of artificial fillers is a lunchtime approach to facial rejuvenation. In most cases, if a patient likes the results achieved with the temporary facial fillers they will be thrilled with the soft, natural and more permanent results that can be achieved surgically with facial fat grafting.

    Because facial fat grafting allows for the placement of fat in specific areas of the face it is truly akin to sculpting a face with a patient’s own fat cells. Other procedures that are often combined with fat grafting to achieve facial reshaping include chin implants, liposuction of the neck and buccal fat pad removal.  All of these procedures involve incisions inside the mouth or in areas not visible or detectable to everyday observers. In this way, facial reshaping can be achieved essentially “scarlessly.”

    Facial Shape Types & Procedures

    •    A “Gaunt Face” or thin face is one on which the fat in the cheeks has disappeared due to heredity, weight loss, disease, or aging. Volume can be added back to the gaunt face though fat grafting into the mid-face and cheek hollows thereby restoring a more healthy and youthful contour to the face.

    •   A “Long Face” is one that typically has a narrow width as well as hollowing in the temporal region and a lack of cheekbone prominence. The temporal areas and cheeks can be augmented with fat grafting in order to increase the width of the face and give a more oval or heart shaped appearance to the facial dimensions.

    •    A “Bottom Heavy Face” is one in which the patient has a “full” lower face due to jowling, excessive submental fat and prominent buccal fat pads. The goal of facial reshaping in these patients is to literally turn the face “upside down” by adding volume to the mid-face and cheeks while simultaneously utilizing liposuction of the neck and removal of the buccal fat pads to “lighten” the lower face.

    •    A “Chubby Face” is one in which a patients complains that their face makes them appear to be overweight.  Similar to the “Bottom Heavy face” facial fat grafting to the cheeks and removal of the buccal fat pads can give these patients the appearance of significant weight loss even though their weight has remained stable.

    •    An “Asymmetric Face” can be caused as a result of heredity, trauma or certain medical disorders. Asymmetric fat grafting can be performed as an alternative to traditional reconstructive treatments in order to rebuild and provide the appearance of improved symmetry of a patients facial features without the drawback of prolonged recovery, scarring or significant financial expense.

    •      In the “Aging Face” we see loss of facial fat volume that leads to a deflated and sunken appearance in the midface, lips, nasolabial folds and underneath the eyes. Fat grafting can be used to replace the fat that has been lost in these areas due to the natural aging process. The results can be quite dramatic as well as being very natural, soft and realistic in appearance.

    Recovery
    Some patients report mild bruising and swelling after the procedure. The majority of the swelling and bruising will be gone in the first week. Because there are no visible incisions one the face, patients can return to normal activities very quickly and resume showering the day after surgery and exercise within the first week. Patients should be cautious with excessive cardiovascular exercise in the fat burning range during the first week after surgery in order to give the fat its best chance to “take” and last long term.

    Conclusion
    Facial fat grafting will continue to increase in popularity over the next decade as more surgeons and patients realize its true potential in facial reshaping.

    I currently use facial fat grafting in place of extensive reconstructive facial surgery and temporary injectable fillers whenever it is feasible due to its reliable results and minimal risk to my patients. In my hands, I can use facial fat grafting to reshape a patient’s face without the need for scars or extensive downtime. I also frequently use facial fat grafting to enhance my surgical results of other common facial aesthetic procedures such as face lifting and rhinoplasty.

    There is certainly a learning curve to master effectively harvesting, purifying and injecting the fat grafting material successfully and patients interested in having facial fat grafting should consider going to an surgeon that has a depth of experience in performing this amazing technique.

    Lastly, I strongly believe that the high content of stem cells contained within the fat grafting material has rejuvenative effects on the facial skin of a patient from the inside out. Since fact grafting is indeed viable, living tissue and not simply an “artificial filler” it only makes sense that it would possess the ability to restore and revitalize the skin from under which it was placed during surgery.
