    Botox is one of the widely popular non-surgical treatments to reduce the appearance of lines and minor wrinkles on the face. The chemical relaxes the muscle, causing the wrinkle to disappear.

    The treatment is effective in reducing frown lines, furrows on the forehead, crow’s feet, and skin bands on the neck. The procedure can be performed in less than hour, and involves almost no downtime.     Unlike other Plastic Surgeons, Dr. Motykie has several surgical specialties and can you give you the makeover you have always wanted from head to toe! A perfect example of this is a recent patient of Dr. Motykies that is more than thrilled with her Rhinoplasty (Nose Job), Liposuction, and Breast Augmentation results…

    This patient is a 21 year old female that underwent a Rhinoplasty for nasal reshaping that included overall reduction in nasal size, correction of a hanging columella and deviated septum as well as reduction in a prominent dorsal “hump” and tethered upper lip.     Where should you have your plastic surgery procedure performed?

    Not all surgeries should be performed in a hospital. In fact, the majority of elective plastic surgery procedures are performed in an office-based facility. If your surgeon is going to perform your procedure in one of these types of facilities, you should check at least the following in order to make sure your surgery is being performed in a state-regulated, safe surgical facility.
    1.    The Facility: Where your surgical procedure is going to be performed should be accredited and in good standing with at least one of the major accredited organizations.
    a.    AAAASF (www.aaasf.org)
    b.    JCAHO (www.jcaho.org)
    c.    AAAHC (www.aaahc.org)

    2.    Accreditation Standards:
    a.    The medical staff must meet strict qualification standards
    b.    The surgery center must pass regular inspections for safety,  sterility  and record-keeping
    c.    The surgery center must pass a peer-reviewed quality-assessment inspection
    d.    The surgery center must meet certain guidelines set forth for emergency equipment and the staff must be trained in advanced cardiac life support and lifesaving protocols

    Remember, hospitals are big, expensive and literally full of sick people. They are therefore NOT the ideal place to have elective, plastic surgery. An office based surgery center offers privacy, comfort, quality staff and quality care.     14.6 Million Cosmetic Plastic Surgery Procedures Performed in 2012
    According to statistics released by the American Society of Plastic Surgeons (ASPS), 14.6 million cosmetic plastic surgery procedures, including both minimally-invasive and surgical, were performed in the United States in 2012, up 5 percent since 2011.

    Cosmetic minimally-invasive procedures increased 6 percent, with more than 13 million procedures in 2012. The top five minimally-invasive procedures were:

    • Botulinum toxin type A (6.1 million procedures, up 8 percent)
    • Soft tissue fillers (2 million procedures, up 5 percent)
    • Chemical peel (1.1 million procedures, up 2 percent)
    • Laser hair removal (1.1 million procedures, up 4 percent)
    • Microdermabrasion (974,000 procedures, up 8 percent)

    Cosmetic surgical procedures with 1.6 million procedures in 2012. The top five surgical procedures were:

    • Breast augmentation (286,000 procedures)
    • Nose reshaping (243,000 procedures)
    • Eyelid surgery (204,000 procedures)
    • Liposuction (202,000 procedures)
    • Facelift (126,000 procedures)

    Like the weather, plastic surgery is always hot in Beverly Hills. The current philosophy really seems to be “less is more” and most patients are requesting a more natural look after surgery. Overall, the day of the “extreme makeover” is over and patients want to simply look like a younger, rested version of themselves rather than undergoing a radical transformation. The reality is, we’ll never reverse time but, we can certainly use anti-aging tools to look as good as we can for any given age. Plastic surgery is becoming part of the arsenal of things that a person can have access to as they consider how they’re going to personally battle the aging process. The plastic surgeon now plays the role of a guide through the maze of numerous procedures an office may have to offer nowadays — whether they are minimally invasive, noninvasive, or surgical.

    Let’s start with the face. A facelift has long been the traditional method of neck and face rejuvenation. It is still an extremely common procedure and most patients request a natural, rested look rather than an over-tightened or “done” look. A mini-facelift allows for youthful contouring of the neck and jowls and it is commonly combined with facial fat grafting to achieve volume restoration as well as skin lift and tightening. The advantage of using fat grafting is the fact that fat has a rich supply of stem cells which are the rejuvenating powerhouse of the human body. There is also a plethora of injectable products that are now available for non-surgical facial rejuvenation that can be used for filling lines and depressions caused by aging. Some of the newest injectables are being engineered to last longer and to contain an anesthetic agent to “numb” the injection site. Lastly, lasers to resurface the facial canvas are constantly being developed and tested. Intense pulsed light can be used to reverse sun damage and fractioned lasers can be used to tighten skin with very little downtime necessary for healing.

    As far as the breasts, silicone is back! Silicone breast implants are now approved by the FDA for use in breast augmentation in all patients over the age of 22. However, the modern techniques in breast augmentation have made the age old question of “silicone vs saline?”somewhat inconsequential. Either implant can be used to create a natural, soft breast and neither implant is “better.” Having said that, there are definite differences between the two implants and these differences need to be discussed with every patient in order to chose the “right” implant in each individual case. Even though breast implants are well-established as being safe and effective in breast augmentation, fat grafting with stem cells seems to hold promise in this area of plastic surgery as well. In the future, we will hopefully be able to manipulate stem cells harvested from a patient’s own fat to reconstruct or augment their breasts!

    In regards to the body, laser technology has now spilled over into the liposuction arena. There are many different laser-assisted liposuction techniques that are now available and there are many more currently in development. The laser is actually inserted under the skin in order to melt the fat before it is removed with the suction cannulas! The laser is believed to improve surgical outcomes by making the procedure “smoother” and less traumatic to the tissue.

    As for treating the skin, the pace of change in the technologies is incredibly fast. There are constant advances in laser- and light-based treatments and the applicability of neurotoxins and injectable fillers such as Botox® (botulinum toxin type A), Restylane® and Juvederm® is always expanding. For example, Botulinum toxin can now be used to erase lines and wrinkles in the forehead, eyes, around the mouth, in the neck and to reduce excess sweating in the armpits and hands! Imagine if this miracle drug could be applied in a nightly cream. Well it might be in the future since current research is showing that the botulinum toxin molecule could potentially be modified so it could be applied as a facial cream instead of given by injection! The potential uses of these neurotoxins and injectables are limitless and we will undoubtedly have all sorts of new variations of these materials in the future. Lastly, a product that is becoming more and more popular is Latisse®. This product promises to grow eyelashes longer and thicker and it appears to be effective since the lines to buy it are growing just as fast as the lashes!

    Just remember that when looking for a plastic surgeon to perform any of the time-tested or newly introduced procedures, please follow a few simply rules to help you achieve the outcomes you desire.

    1. Safety first: Always look for a surgeon that will place your personal safety and health before anything else. These are elective surgical procedures that are aimed at increasing your beauty and quality of life but they can all come with significant risks if performed by the wrong hands. A safe plastic surgeon should alway follows his oath as a doctor to “Do No Harm” as his primary directive.

    2. Board Certification: Always do your homework and make sure that your surgeon is a Board Certified Plastic Surgeon. Being simply “board Certified” is not enough. Always make sure to ask your surgeon what they are board certified in rather than if they are board certified. For example, a surgeon performing plastic surgery can be Board certified in almost anything such us Board Certified in Dermatology, Board Certified in Gynecology, Board Certified in Cosmetic Surgery, Board Certified in Facial Plastic Surgery, etc… but only true plastic surgeons are Board Certified in Plastic Surgery (ABPS).You can look for this symbol displayed on their webpage or (you can check on your doctors Board Certification in Plastic Surgery by following this link. Board Certification in plastic surgery is important because it means that the doctor passed intensive and extensive training and testing that provided proper training, knowledge, ethics and safety in performing all current plastic surgery procedures. It is a process that takes many years to complete and it is never truly done since the American Board of Plastic surgery requires all of its Diplomats to continue educating themselves and reporting their surgical cases to them on a regular basis. The surgeons are also tested on a routine basis in order to assess their continuing medical education and knowledge base of current trends and procedures in plastic surgery procedures.

    3. Surgical Talent: One thing the Boards do not evaluate is the individual surgical and artistic talent of any given surgeon. This is one thing you need to do on your own. The best way to assess a surgeon’s own personal aesthetic and talent is to review their online photo galleries of past patients. Review both the quality and quantity of work. IN plastic surgery, beauty is definitely in the eye of the beholder, and you must take the time to see if your vision of beauty matches any potential surgeon’s aesthetic sense before every stepping in to their operating room.

    4. In person Consultation: Lastly, an in person consultation is critical to make sure that a surgeon’s ego does not get in the way of you getting the results that YOU truly want from surgery. A great plastic surgeon will always take the time to listen to your own personal desires and concerns about any possible plastic surgery procedure. Before leaving your consultation make sure that all of your questions have been answered and always make sure that the surgeon and his staff are kind, courteous, welcoming and knowledgeable.

    Using Facial Fat Grafting, Fat Transfer or Adipose Stem Cell Injections in Facial Reshaping

    The Use of Fat Grafting in Facial Reshaping
    Patients who are unhappy with their facial features are often turning to facial fat grafting rather than undergoing complex and costly reconstructive surgeries.  Facial fat grafting is now commonly used in place of injectable artificial fillers and silicone facial implants because most patients have an abundant supply of donor fat tissue and the procedure is essentially scarless.

    Also, Facial Fat Grafting is frequently combined with other facial rejuvenation and aesthetic procedures such as face lifting, lower eyelid surgery and rhinoplasty.  Some common facial motifs that can benefit from facial fat grafting are the gaunt face, long face, bottom-heavy face, chubby face, asymmetric face and the aging face.

    Patients often turn to facial fat grafting as an alternative to undergoing more invasive surgeries because fat grafting can yield a very natural result with a minimal amount of downtown for recovery.

    What Is Facial Fat Grafting?
    Facial Fat grafting (fat transfer, fat injection or adipose stem cell grafting) is a medical procedure that uses a patient`s own fat to rejuvenate or augment certain areas of the face.

    This fat is harvested from a donor site (such as the abdomen, waist, inner thighs or inner knees) utilizing a small syringe or blunt cannula. The fat is purified in a centrifuge in order to separate the blood, fluid and damaged fat cells prior to its use in facial grafting. The fat is then placed in small syringes and injected into the predetermined areas of the face.

    Facial aging is related to not only loose skin in the face and neck, but also to a loss of facial fat volume over time. As we age, we lose the fat underneath our eyes and in our cheeks which can lead to a gaunt, tired look even though we feel very much young and alive. A face lift is used to tighten and remove the excess skin in the face while fat grafting is utilized to replace the fat that has been lost during the aging process. The fat that has disappeared with age is easily replaced to create the most natural and youthful facelift result available from plastic surgery today.

    Because the fat graft material contains stem cells it allows for a minimally invasive rejuvenation procedure that gives the plastic surgeon the ability to sculpt and restore an aging face to a more youthful appearance.

    Fat grafting is an extremely versatile plastic surgery modality. Once the fat has been harvested and purified, it can be used to accomplish aesthetic facial reshaping by accentuating the cheekbones, filling in facial defects or scars, plumping the lips, camouflaging dark circles underneath the eyes and improving overall facial contour Fat grafting can also be used to take the place of facial implants without the associated risks of implants such as migration, scarring, infection and extrusion.

     

    Facial Reshaping Is Truly Sculpting Facial Features
    Throughout the evolution of plastic surgery, facial reshaping has traditionally been accomplished with reconstructive procedures and placement of silicone facial implants that can become very complicated surgeries with extended recovery periods.

    With the recent popularity of artificial fillers such as Restylane and Juvederm, more attention has been directed to restoring facial volume. Although many doctors encourage patients to use these products to reshape the face, patients need to be aware that these corrections are temporary, and often several syringes are needed to fill in larger depressions. This can lead to considerable expense over time that may end up costing more than having facial fat grafting in the first place.

    Facial fat grafting offers an alternative to facial fillers for those patients that do not want to spend a lot of money on temporary artificial fillers that require yearly or bi-yearly maintenance. The main advantage of artificial fillers is a lunchtime approach to facial rejuvenation. In most cases, if a patient likes the results achieved with the temporary facial fillers they will be thrilled with the soft, natural and more permanent results that can be achieved surgically with facial fat grafting.

    Because facial fat grafting allows for the placement of fat in specific areas of the face it is truly akin to sculpting a face with a patient’s own fat cells. Other procedures that are often combined with fat grafting to achieve facial reshaping include chin implants, liposuction of the neck and buccal fat pad removal.  All of these procedures involve incisions inside the mouth or in areas not visible or detectable to everyday observers. In this way, facial reshaping can be achieved essentially “scarlessly.”

    Facial Shape Types & Procedures

    •    A “Gaunt Face” or thin face is one on which the fat in the cheeks has disappeared due to heredity, weight loss, disease, or aging. Volume can be added back to the gaunt face though fat grafting into the mid-face and cheek hollows thereby restoring a more healthy and youthful contour to the face.

    •   A “Long Face” is one that typically has a narrow width as well as hollowing in the temporal region and a lack of cheekbone prominence. The temporal areas and cheeks can be augmented with fat grafting in order to increase the width of the face and give a more oval or heart shaped appearance to the facial dimensions.

    •    A “Bottom Heavy Face” is one in which the patient has a “full” lower face due to jowling, excessive submental fat and prominent buccal fat pads. The goal of facial reshaping in these patients is to literally turn the face “upside down” by adding volume to the mid-face and cheeks while simultaneously utilizing liposuction of the neck and removal of the buccal fat pads to “lighten” the lower face.

    •    A “Chubby Face” is one in which a patients complains that their face makes them appear to be overweight.  Similar to the “Bottom Heavy face” facial fat grafting to the cheeks and removal of the buccal fat pads can give these patients the appearance of significant weight loss even though their weight has remained stable.

    •    An “Asymmetric Face” can be caused as a result of heredity, trauma or certain medical disorders. Asymmetric fat grafting can be performed as an alternative to traditional reconstructive treatments in order to rebuild and provide the appearance of improved symmetry of a patients facial features without the drawback of prolonged recovery, scarring or significant financial expense.

    •      In the “Aging Face” we see loss of facial fat volume that leads to a deflated and sunken appearance in the midface, lips, nasolabial folds and underneath the eyes. Fat grafting can be used to replace the fat that has been lost in these areas due to the natural aging process. The results can be quite dramatic as well as being very natural, soft and realistic in appearance.

    Recovery
    Some patients report mild bruising and swelling after the procedure. The majority of the swelling and bruising will be gone in the first week. Because there are no visible incisions one the face, patients can return to normal activities very quickly and resume showering the day after surgery and exercise within the first week. Patients should be cautious with excessive cardiovascular exercise in the fat burning range during the first week after surgery in order to give the fat its best chance to “take” and last long term.

    Conclusion
    Facial fat grafting will continue to increase in popularity over the next decade as more surgeons and patients realize its true potential in facial reshaping.

    I currently use facial fat grafting in place of extensive reconstructive facial surgery and temporary injectable fillers whenever it is feasible due to its reliable results and minimal risk to my patients. In my hands, I can use facial fat grafting to reshape a patient’s face without the need for scars or extensive downtime. I also frequently use facial fat grafting to enhance my surgical results of other common facial aesthetic procedures such as face lifting and rhinoplasty.

    There is certainly a learning curve to master effectively harvesting, purifying and injecting the fat grafting material successfully and patients interested in having facial fat grafting should consider going to an surgeon that has a depth of experience in performing this amazing technique.

    Lastly, I strongly believe that the high content of stem cells contained within the fat grafting material has rejuvenative effects on the facial skin of a patient from the inside out. Since fact grafting is indeed viable, living tissue and not simply an “artificial filler” it only makes sense that it would possess the ability to restore and revitalize the skin from under which it was placed during surgery.
